CLEARANCE PROCESS | These documents and processes are required for
participation in athletics, and are to be completed/submitted to the FHS
Athletic Department prior to participation in summer sports, Fall semester
off season practices, or tryouts for any given school year.
- Undergo your physical using this form.
- Register for an account and/or log in here to start your clearance process for Foothill/Santa Ana (CIF-SS). If you have trouble uploading the physical form, bring it to the FHS Athletic Office in room 303. A physical is valid for one year, and a new physical is required before the expiration date for athletes to continue playing.
- Complete the Concussion Baseline Test from https://www.xlntbrain.com/ by following these directions.
